Application Documents
Please fill out and submit the above application before sending your supporting documents. Once submitted, please email the following items to foundation@forumcu.com. Copies (including pictures) of all documents are acceptable. (The documents do not have to be sent from your schools.)
- High school report card showing GPA (if currently in high school) or current college transcript showing GPA (if currently in college)
- College/university acceptance letter or current college enrollment verification showing the number of hours taking. (If you have not yet been accepted to a school, then this is not required.)
- Signed consent and media release form. (Submit a scan or picture of the signed form)
- Please note, for the security of our members, we do not accept Dropbox or Google Docs, and will only view videos that are submitted via public YouTube link.

Scholarship applications and supporting documents are due each year no later than February 1. Students will be notified of their application status no later than March 31.

Relatives of FORUM Credit Union officers (anyone holding the position of Assistant Vice President, Vice President or Chief Executive), the board of directors and supervisory committee members are not eligible to apply for collegiate scholarships. Relatives of non-officer FORUM Credit Union employees may apply for the collegiate scholarships.
