If you did not attach the following items to your form with your application, please email them to foundation@forumcu.com. Copies (including pictures) of all documents are acceptable.

• Signed consent and media release form. (Submit a scan or picture of the signed form)
• Please note, for the security of our members, we do not accept Dropbox or Google Docs.
